Therapy provider Bexar County has projected financial losses
under the deduction of Medicaid
After Texas moved forward by cutting Medicaid replacements
to therapists working with disabled children, service providers in Bexar County
projected losses in hundreds of thousands of dollars.
Reimbursement of fees under the country's Medicaid program
includes physical, occupational and speech therapy for disabled children. The
previous state legislature agreed to cut about $ 350 million, although now some
legislators, including House Speaker Joe Straus, are working on a plan to
reverse it.
Kim Jefferies, chief executive officer of the Brighton Center,
one of three providers of Early Childhood Intervention in Bexar County, said
that of the 1,700 children being served, 1,139 were registered with Medicaid.
ECI provides free or discounted therapy for infants and toddlers with
developmental disorders, such as speech problems, Down syndrome and autism.
"(Withholding) will translate into a loss of income of
approximately $ 250,000," Jefferies said, starting from December 15, when
the deductions took effect, until August 31. "We have to find ways to make
that income, including community support, foundation support and individual
support. We have to raise these funds to support that every eligible child gets
the services they need."
Medicaid reimbursement cuts take effect in December, more
than a year after MPs agreed. Some health care providers at home and also
parents demanded to try to stop the deduction, which they said affected as many
as 60,000 children across the state. Opponents say that lower replacements will
force some providers to restrict services or close down, moving more children
to fewer therapists.
The court rejected the claim, and also found that the state
acted in its authority to set tariffs.
Regardless of how much replacement they get for service,
Jefferies said they still had to provide therapy.
"This puts us in a unique situation where we were asked
by the state of Texas in our contract to provide services for each child, but
the state cut our funds to do that," Jefferies said.
State legislators have cut funding for ECI by 11 percent in
the past seven years, according to a report by the advocacy group Texans Care
for Children. As a result, ECI registration declined, it was found. Texas
ranked 43rd nationally in 2014 for the percentage of children enrolled in the
ECI, and since 2011, the number of ECI contractors in the state declined from
58 to 47.
Other Bexar District ECI programs are provided by the Health
Care Service Center, which serves around 700 children, including those who have
family relationships or foster care. About 500 of these children are Medicaid
recipients.

Zaida Yzaguirre, program director, projected a loss of
income of approximately $ 83,600 until August 31 due to cuts and said that
layoffs were a possible way to overcome the income gap.
"We are running a very slim engine," Yzaguirre
said. "We serve all children who fulfill the requirements without being
differentiated. We serve them all because we have to comply with the law.
"
With state revenues squeezed by declines in the oil or gas
industry, the DPR budget writers and the Senate have taken very different
positions in the replacement level controversy. Straus completed a plan to
reverse additional budget cuts for the two-yearly legislature, but the Senate
leaders have not.
At a Senate Finance Committee hearing on Tuesday, where
parents of crying children with delayed development of witnesses, Sen. Jane
Nelson, R-Flower Mound, said there was never any intention of legislators to
take services.
He said that cuts were based on information indicating that
Texas tariffs were far above those in other states.
Marty Drake brought his 16-month-old daughter, Sully,
diagnosed with Down syndrome. He said that he and his wife had been advised by
one of the therapists in Livingston that if the cut was not reversed, the
provider would not remain open and Sully would not receive occupational
therapy.
Drake said that without service at Livingston, they had to
go to another center in downtown Houston, about 70 miles away.
Nelson said the Texas Health and Human Services Commission
should consider access to care when applying tariffs. "We will ensure, in
any case, that no child is denied service," he said.
"It makes me very sad that people are so scared,"
he said. "There is no service to cut."
After hearing from parents, State Senator Carlos Uresti,
D-San Antonio, deputy chairman of the Health and Humanitarian Services
Committee or a member of the Finance Committee, said: "I hope that the
Legislature will acknowledge and agree that we need to put the money back into
the budget. "
